"[The following project information shall be submitted to the Township for every proposed solar-energy facility:] (2) An affidavit or similar evidence of agreement between the property owner and the solar-energy facility owner or operator demonstrating permission to apply for necessary permits for construction and operation of a solar-energy facility.
(3) Identification of the properties on which the proposed facility will be located and the properties adjacent to the proposed location."
